The Illumination Foundation is currently seeking EMT or Medical Assistant (Medical Coordination Assistant) at our Recuperative Care Centers in Buena Park.
The Illumination Foundation (IF) is a non-profit 501(c)(3) organization that addresses the crisis of homelessness in Orange County through integrated services that provide appropriate solutions.
Medical Coordination Assistant is responsible for ensuring that the medical care and daily tasks are addressed for homeless clients discharged from hospitals or other agencies contracted with IF Recuperative Care Center are being properly completed to the standards of IF Managers/Supervisors/Directors/Board of Directors.
Complete medical checks
Initial assessment of emergency situations
Ensure client has a successful hospital discharge
Verify follow-up appointments and/or schedule appointments
Home health coordination
Teach clients on illnesses, preventative measures, and overall general health with supervision of Medical Coordinator
Maintain client compliance to program rules and regulations
Transport clients to and from appointments as needed
Preparation of clients' living arrangements
Advocate for clients to ensure best medical care is provided
Maintain client files
Maintain proper inventory of medical equipment and supplies for site
Attend all IF staff, recuperative meetings and trainings as requested
Must be able to make decisions, meet targets and work under pressure
Knowledge of medical terminology, diagnosis and medications
Demonstrated critical thinking and leadership skills
Strong communication and negotiating skills; ability to build and maintain strong patient relationships
A sense of urgency, comfort with ambiguity, strive for results and ability to remain true to the company's brand and image
Preferred Experience/ Minimum Qualifications:
High School Diploma or equivalent
Current and non-restricted EMT, CNA, or MA certification
Associates degree preferred
One year of relevant experience
Customer and client management
Strong self-management skills
